Program: HELP One Building A | 273 Snediker Avenue, Brooklyn, NY 11207 What You'll Do As a Clinical Case Manager with HELP USA, you’ll provide case management and crisis intervention services to residents in our Brooklyn supportive permanent housing programs with a goal of promoting independent living skills, increased housing stability, and well-being. Your responsibilities will include: Conducting assessments, formulating individualized service plans, and providing referrals and on-going support to assist tenants in developing skills and achieving personal goals associated with housing stability and well-being. Serving as the foundation of the supportive services made available to tenants such as connecting them to community-based services and resources that are aligned with their ISP. Facilitating a range of support groups and workshops designed to help tenants live as independently as possible. Serving as a liaison with medical, mental health, and substance abuse providers to ensure a level of care that the consumers deserve. Providing assistance with educational and employment linkages. Helping tenants overcome barriers that may affect the timely submission of monthly rent. Work with property management to support tenants in all aspects of maintaining permanent housing. Advocating for and with residents in all aspects of their lives and offering (but not imposing) assistance. You’re a great fit for this role if you have: Bachelor's Degree in Social Work, or a related field required. CASAC certification desired, but not required. Minimum two (2) years’ experience in delivering clinical crisis services. Experience in case management, assessment, counseling and crisis intervention preferred. Preferred experience working with families with children. Experience in Motivational Interviewing (MI), Wellness Recovery Action Plan (WRAP) and Person Centered Planning (PCP) preferred. Knowledge of AWARDS a plus. Computer literate, specifically with Microsoft applications, required. Compensation Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws. We Have GREAT BENEFITS!
