Company Description We design and disseminate modern models of care for high-need older adults and caregivers delivered by an expert, empowered multi-disciplinary team. The Role We are a telehealth practice dedicated to supporting people living with dementia and their caregivers on the foundation of the GUIDE Dementia Program. We are seeking a versatile Operations & Integration Specialist for a remote, flexible role. This role will be a foundational "builder" responsible for the expeditious execution of our mission, managing the operational, clinical, corporate, and vendor infrastructure required to deliver high-touch dementia care and specialized telehealth services in FL and select other states. Core Responsibilities This position leads growth across five primary domains: • Comprehensive Program Management: Oversight of dementia program standards and internal care models. This involves ensuring clinical workflows and support services are seamless, efficient, and fully compliant with regulatory requirements. • Strategic Growth & Partner Development: Management and activation of referral pathways to expedite dissemination of dementia services. This includes partner relationship management and cultivating new B2B relationships across the healthcare continuum. • Expansion Infrastructure: Coordination of the logistical requirements for multi-state expansion. This includes managing the administrative processes for insurance credentialing, clinician licensing, and corporate filings to ensure operational readiness. • Platform Diversification: Operational launch of specialized service lines beyond the core dementia model. • Vendor Relations & Logistics: Identification and management of third-party service providers. This includes overseeing the logistical and payment systems necessary to maintain high standards of service delivery across all vendor partnerships. Qualifications • Startup Experience: Ability to thrive in a dynamic healthcare startup environment as a proactive problem-solver. • Sector Expertise & Mission Alignment: Significant experience working within clinical or operational models specifically serving high-needs older adults. We are seeking a candidate dedicted to improving the lives of older adults, their families and caregivers, and the healthcare workers who serve them. • Operational Tool Fluency: High proficiency with modern project management, AI, CRM, and healthcare software. • Sector Expertise: Experience in telehealth operations, Medicare care models, or healthcare vendor management. Familiarity with the administrative landscape of supporting high-needs older adults is preferred. • Clinical Background (Valued): A background or licensure as an RN, NP, PA, or Social Worker is valued, but not required.
