LSW Field Care Manager Behavioral Health HealthCare Support is actively seeking multiple Licensed Social Worker Case Managers to fill multiple openings with a Managed Care Organization in Wayne and Macomb Counties. Daily Responsibilities for LSW Field Care Manager Behavioral Health: • Assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. • Shift: M-F 8AM-5PM • This is a HYBRID position with Heavy Field Work and Remote Work Required Qualifications for LSW Field Care Manager Behavioral Health: • Licensed Bachelor's prepared Social Worker or continuous active limited social worker license with intent to obtain full licensure. • LMSW • Minimum of 2 years of post-degree clinical experience in behavioral health setting • Michigan State licensure in field of study Preferred Qualifications for LSW Field Care Manager Behavioral Health: • Ability to travel to region-based facilities and homes for face-to-face assessments • Experience working with the adult population, disease management. • Knowledge of community health and social service agencies and additional community resources.
