Outreach Coordinator – IIH & CST Services Vaya Catchment area ONLY About Us: Greater Outreach Services, Inc. supports individuals with behavioral health needs through recovery-focused programs. We provide Intensive In-Home (IIH) and Community Support Team (CST) services to empower clients and families in the Vaya Health catchment area. Position Overview: We are seeking a proactive Outreach Coordinator to generate and manage referrals for IIH and CST services. The ideal candidate has a proven track record of building community partnerships, engaging referral sources for Medicaid recipents and increasing service utilization. Key Responsibilities: • Actively identify and engage referral sources within the Vaya catchment area. • Generate and track referrals for IIH and CST programs. • Build relationships with hospitals, schools, clinics, and community agencies. • Support clients and families in accessing services and navigating care. • Provide outreach presentations and participate in community events. • Collaborate with IIH and CST teams to ensure smooth intake and service delivery. Qualifications: • High school diploma, Bachelor’s degree in Human Services, Social Work, Psychology, or related field. • Minimum 2 years of behavioral health outreach or care coordination experience. • Demonstrated success in generating active referrals. • Knowledge of IIH and CST services and the Vaya catchment area preferred. • Strong communication, networking, and organizational skills.
