We're Hiring: Care Manager (I or II) Location: Grapevine, TX - in office Department: Client Services Reports to: Client Services Manager Status: Full-Time, Exempt GRACE is seeking a compassionate and adaptable Care Manager to join our Client Services team. This position may be filled at either the Care Manager I or II level depending on experience and qualifications. The Care Manager provides intake and service delivery to individuals and families in need, with Care Manager II assuming additional responsibilities for complex case management, housing coordination, and departmental support. Essential Duties and Responsibilities:All Care Managers will: Interview clients and determine eligibility for assistance at GRACE Client Services locations. Administer aid per agency guidelines and document services in both paper and electronic formats. Serve as interpreter for Spanish-speaking clients to ensure equitable access to services. Care Manager II will additionally: Conduct complex case management for clients facing homelessness or housing instability. Coordinate emergency services with community partners and referral networks. Determine eligibility for GRACE housing programs (MH, TBLA 114, CBG) and execute referrals. Utilize systems such as HMIS and Data Warehouse to assess placement needs and chronicity. Respond to client concerns and mentor Care Manager I staff in the absence of the Client Services Manager. Assist peers with challenging client scenarios to ensure consistent support across the department. Required Qualifications: Bachelor’s degree in social work, human services, or a related field. Two to three years of case management. Bilingual in English and Spanish, professionally. Strong interpersonal, organizational, and communication skills.
