Overview We are seeking a dynamic and dedicated HCBS Coordinator/Receptionist to join our team! This vital role combines front desk responsibilities with coordination of Home and Community-Based Services (HCBS), ensuring smooth communication, organization, and support for our clients and staff. The ideal candidate will be energetic, highly organized, and possess excellent customer service skills. You will serve as the first point of contact for visitors and callers while managing administrative tasks that keep our operations running seamlessly. HCBS Coordinator/Bilingual $19-$21/hr • Develop and implement individualized support plans, identifying necessary services and resources to meet clients' needs. • Conduct assessments to determine eligibility for services and identify support needs. • Coordinate with direct support professionals, healthcare providers, and other stakeholders to ensure comprehensive care. • Advocate for clients' rights and interests, ensuring they receive necessary accommodations and services. • Maintain accurate records, prepare reports, and document client progress. • Build relationships with community resources, healthcare providers, and other stakeholders to expand service offerings.
