Provide Switchboard Coverage for all Maternal and Child Health Programs a. Complete Children 1st Screening and Referral Form in Person or over the Telephone b. Direct Phone calls to the appropriate programs (Babies Can’t Wait, Children 1st, First Care, Early Hearing Detection and Intervention, and Children Medical Services) 2. Schedule appointments a. Call Clients within 2 business days of receiving a referral. b. Document phone call attempt and all other contact attempts on a Progress Note c. Assessments and visits should be complete within 14 days of receiving a referral d. If appointment scheduled, enter client’s demographic information on staff’s Outlook Calendar e. Place referral with time and date of appointment in staff’s mailbox f. If unable to contact client by phone i. Send an introductory letter to client with 2 days of the phone call ii. Attempt a second phone call within 7 days of initial phone call. iii. Call secondary phone number iv. Contact referral source to seek alternate contact information for client 1. If referral submitted by DFCS, contact caseworker by phone and/or email and send an email to HealthMatters v. If client does not contact staff, send a 10 day notice that case will be closed 3. Process referrals that are received via fax, email, or in-person a. Copy immediately and distributed to the appropriate program as determined in the Birth to Five staffing meeting b. Maintain a copy of the referral to be entered into the State Electronic Notifiable Disease Surveillance System (SENDSS) database 4. Send correspondence to referral source a. Mail thank you letter to referral source 5. Enter data into the State Electronic Notifiable Disease Surveillance System (SENDSS) (Within 2 business days of receiving a referral) a. Enter Children 1st Screening and Referral Form (See SENDSS Training Manuel) i. Open Disposition in SENDSS 1. Risk Factors identified (Children 1st Program) 2. Refer to other programs (BCW, CMS, EHDI, or First Care) 3. Scan completed referral form into SENDSS 4. File refer form by dates b. Ages and Stages Developmental Screening Tool i. Enter completed Ages and Stages Questionnaire into SENDSS database ii. Upload entered Ages and States Questionnaire into SENDSS c. Modified Checklist for Autism in Toddlers (MCHAT) i. Contact Physician Office to receive MCHAT completed on a client between 16 to 30 months of age ii. Enter data from completed MCHAT into SENDSS iii. Upload completed MCHAT from into SENDSS d. Maternal Child Health (MCH) Assessment i. Enter completed MCH Assessment ii. Upload completed MCH into SENDSS 6. Attend Outreach Activities/Health Fairs a. Attend at least 2 Programmatic Related Outreach Activities High school diploma/GED and two (2) years of experience communicating information in a customer service setting. OR One (1) year of experience required at the lower-level Customer Svc Rep 1 (GST120) or position equivalent NOTE: Some positions may require area specific certifications/licenses.
