Role Overview: As a Case Manager at Geisinger, you will play a vital role in coordinating patient care within the Hospital at Home program. Your focus will be on ensuring seamless care transitions, optimizing patient outcomes, and collaborating with multidisciplinary teams to deliver high-quality healthcare services. What You'll Do: - You will assess patient needs and develop individualized care plans. - You will coordinate with healthcare providers, patients, and families to facilitate effective communication. - You will monitor patient progress and adjust care plans as necessary. - You will ensure compliance with healthcare regulations and organizational policies. - You will provide education and support to patients and their families. - You will document all case management activities accurately and timely. - You will participate in quality improvement initiatives to enhance patient care. - You will collaborate with community resources to support patient needs post-discharge. What You Bring: - You bring a current RN license with at least 3 years of clinical nursing experience. - You bring strong knowledge of case management principles and healthcare regulations. - You bring excellent communication and interpersonal skills. - You bring proficiency in electronic medical records (EMR) systems. - You bring the ability to work independently and as part of a multidisciplinary team. Bonus Points If You Have: - Experience in hospital at home or home health care settings. - Certification in Case Management (CCM) or related credentials. - Familiarity with quality improvement methodologies. - Bilingual skills to support diverse patient populations.
