Full Description

FlexStaff is seeking a Full-Time Field LPN to conduct home visits to patients in Westchester County, NY with one of our clients, a multi-service home care organization. The role offers a schedule of Monday to Friday, 38.5 hours per week. A car and valid driver’s license are required. A candidate must reside in Westchester County. At least 1 year home care or assisted living facility experience required. Responsibilities include, but not limited to: • Patient Assessment: Conduct initial assessments and ongoing evaluations of patients' health status, including monitoring vital signs. • Medication Management: Administer prescribed medications and manage medication schedules, ensuring compliance with treatment plans. • Personal Care: Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting. • Wound Care: Provide care for wounds, including dressing changes, cleaning, and monitoring for signs of infection. • Patient Education: Educate patients and their families about medical conditions, treatment plans, and home care techniques. • Care Coordination: Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans. • Documentation: Maintain accurate and up-to-date patient records, documenting care provided, patient progress, and any changes in condition. • Hygiene and Safety: Ensure a clean and safe environment for patients, adhering to infection control standards. • Emotional Support: Provide emotional support and companionship to patients, addressing their psychological well-being as part of holistic care.
