Job Title: Midlevel Provider (Nurse Practitioner / Physician Assistant) Location: Multiple Sites – Hospital & Assisted Living Facilities Employment Type: Full-Time Position Summary: We are seeking a skilled and compassionate Midlevel Provider (NP or PA) to join our growing medical team. This position will provide medical care to patients in hospital and assisted living settings, working closely with physicians, nursing staff, and facility leadership to deliver high-quality, patient-centered care. Responsibilities: • Perform comprehensive patient assessments, including history and physical exams • Diagnose and treat acute and chronic medical conditions • Develop and manage treatment plans and follow-up care • Round on patients in hospital and assisted living facilities • Order and interpret diagnostic tests • Prescribe medications and treatments in accordance with state law • Communicate with patients, families, and care teams regarding care plans • Maintain accurate and timely medical documentation in the EMR • Participate in quality improvement and care coordination initiatives Qualifications: • Current Nurse Practitioner or Physician Assistant license in the state • Active DEA license (or ability to obtain) • Hospital and/or long-term care experience preferred • Strong clinical, communication, and organizational skills • Ability to work independently and collaboratively Schedule & Compensation: • Flexible scheduling options available • Competitive salary based on experience • Benefits package may include health insurance, paid time off, and CME Why Join Us: • Supportive clinical team • Opportunity to work in diverse care settings • Stable, growing medical practice Work Location: In person
