Care Manager for Lead Care Manager position (Desired start date: Immediate) Position Overview The Lead Care Manager (LCM) coordinates and implements organization-wide Enhanced Care Management. In this capacity, the LCM oversees and implements the provision of Enhanced Care Management (ECM) services for plan members under this LCM’s care. The services provided under this role are specific to the County of San Luis Obispo and the Northern and Central regions of Santa Barbara County. Applicants should be aware that engagement with these communities will be conducted in person, and must be comfortable traveling within these service areas as part of their responsibilities. If you are passionate about working with underprivileged and marginalized populations, seniors, and disabled adults, helping them to overcome a loss of self-worth, enhance their quality of life, and preserve their dignity and self-esteem, we want to talk to you! The position will allow you to work with the best in your field healthcare professionals who have worked in our industry for many years by providing case management and community supports services. Desired · Degree or Experience in a Social Work or Nursing fields related to work with seniors, disabled adults, minors and youth. · Microsoft Office product expertise required (Excel, Word, PowerPoint, Outlook) · Bilingual · Strong interpersonal and communication skills, with the ability to connect with diverse populations. · Empathy, cultural sensitivity, and a passion for improving community health and well-being. · Excellent organizational skills and attention to detail. · Ability to work independently and collaboratively within a team. · Valid California Driver's license.
