Care Manager for Lead Care Manager position (Desired start date: Immediate) Position Overview The Lead Care Manager (LCM) coordinates and implements organization-wide Enhanced Care Management. In this capacity, the LCM oversees and implements the provision of Enhanced Care Management (ECM) services for plan members under this LCM’s care. The services provided under this role are specific to the County of San Luis Obispo and the Northern and Central regions of Santa Barbara County. Applicants should be aware that engagement with these communities will be conducted in person, and must be comfortable traveling within these service areas as part of their responsibilities.
