PACE, or Program of All-Inclusive Care for the Elderly, is an alternative care model for those over 55 years of age meeting nursing home level of care which aims to keep participants living safely in the community. PACE is a collaborative, interdisciplinary approach which coordinates all levels of medical and social needs, including prevention, wellness, socialization, primary care, and access to specialists and other services. Position purpose: The Personal Care Attendant (PCA) provides assistance and support to individuals in PACE who need help with daily living activities due to disabilities, illness, or age-related conditions. PCAs offer personalized care, ensuring the comfort, safety, and well-being of the participant. PCAs are members of the PACE Interdisciplinary Team and perform under the supervision of the PACE Center Director. Primary Duties and Responsibilities: • Assist participants with ADLs, including bathing, grooming, toileting, dressing, and feeding. • Provide mobility support, including safe transfers and repositioning, utilizing proper techniques and equipment. • Document participant care and observations accurately and in a timely manner. • Report changes in participant condition to the IDT or supervisor. • Prepare and serve meals, adhering to dietary guidelines and participant preferences. • Perform light housekeeping, including laundry, cleaning, and organizing participant areas. • Support participants with medication reminders and management under supervision and as permitted by state regulations. • Engage participants in meaningful activities and provide companionship to enhance their quality of life. • Participate actively in IDT meetings, offering insights about participant needs and progress. • Support the IDT in implementing individualized care plans and interventions. • Maintain participant privacy and confidentiality at all times. • Ensure the environment is safe and hazard-free for participants. • Participate in PACE Quality Improvement initiatives as needed. • Attend mandatory training, in-services, and professional development sessions. • Utilize appropriate personal protective equipment (PPE) and follow infection control protocols Support and Cross-Coverage Responsibilities: • Reception desk coverage, including handling incoming phone calls and intakes • General office work and administrative assistant duties • Assisting with PACE Center activities Esperanza Health Centers is an Equal Opportunity Employer (EOE) according to Title 44, Ill. Applicants must meet all state-specific requirements for unlicensed assistive personnel. • Successful completion of IDPH-required background check, including fingerprinting, and clearance in the Healthcare Worker Registry prior to direct participant care. • Previous experience working with older adults required; minimum one year experience preferred. • Excellent verbal and written communication skills. • Excellent interpersonal skills. • Excellent organizational skills and attention to detail. • Ability to prioritize tasks and consult with licensed staff when tasks exceed scope of practice. • Ability to act with integrity, professionalism, and confidentiality. • Adhere to all regulations and laws regarding scope of practice, privacy, and participant rights. • Proficient with Microsoft Office Suite or related software at basic level.
