Job Description: We are seeking a dynamic and experienced Director of Patient Care Services to join our team. The successful candidate will be a Registered Nurse (RN) with extensive home health experience. This leadership role involves overseeing the clinical operations, ensuring the highest standards of patient care, and maintaining compliance with all regulatory requirements. Key Responsibilities • Lead and manage the patient care services team, including nurses, therapists, and other healthcare professionals. • Develop and implement clinical policies and procedures to ensure high-quality patient care. • Monitor and evaluate patient care services to ensure compliance with state and federal regulations. • Oversee the development and implementation of patient care plans. • Conduct regular staff meetings and provide ongoing training and professional development opportunities. • Collaborate with physicians, patients, and families to coordinate comprehensive patient care. • Manage the performance improvement process to enhance patient outcomes. • Ensure accurate and timely documentation of patient care services. • Serve as a resource and mentor for clinical staff, providing guidance and support as needed. Requirements Qualifications: • Current and unrestricted Registered Nurse (RN) license in the state of CA. • Minimum of 5 years of experience in home health care. • Proven leadership and management experience in a clinical setting. • Strong knowledge of home health care regulations and standards. • Excellent communication, organizational, and interpersonal skills. • Ability to work effectively in a fast-paced and dynamic environment. • Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
