What You'll Be Doing as a Nurse Practitioner/Physician Assistant - In-Home Primary Care (Field-Based Role) • Conduct in-home primary care visits, including comprehensive health assessments and chronic disease management. • Develop and implement individualized care plans for adult patients with acute and chronic health conditions. • Educate patients and families on health management strategies, medication adherence, and preventive care. • Collaborate with a multidisciplinary team to ensure continuity of care and optimal patient outcomes. • Document patient interactions using an EMR system (Athena preferred) to maintain accurate records. What You'll Bring as a Nurse Practitioner/Physician Assistant - In-Home Primary Care (Field-Based Role) • Certified Nurse Practitioner (NP) or Physician Assistant (PA) from an accredited program. • Minimum of 2 years of clinical experience in primary care or home health, with a demonstrated ability to work independently. • Active, unrestricted state license (or eligibility) and a valid driver's license. • Current DEA registration (or eligibility) and Medicare/Medicaid enrollment (or eligibility). • Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS). • Proficiency with EMR systems and familiarity with Microsoft Office. • Strong interpersonal and communication skills, with a commitment to patient-centered care.
