Our residential programs offer a person-centered service that provides innovative and high quality programming to adults with intellectual and developmental disabilities. The program offers support and opportunities for individuals to enrich their lives, develop social and daily living skills, and build social and community connections. The ideal candidate will have experience working with adults with intellectual or developmental disabilities. Responsibilities: • Provide direct medical care as ordered by a physician • Educate and consult with individuals and staff on medical conditions, medications, and general health topics • Deliver individualized training and facilitate health and wellness groups • Assist individuals in accessing and utilizing community health resources • Guide individuals in understanding their medical conditions including symptoms, complications, and treatment options • Review medical documentation and provide care coordination and consultation as needed • Coordinate and follow up on medical care and attend appointments as assigned • Provide side-by-side interventions focusing on health education and skills training • Support the team in assessing symptoms and treatment effectiveness and recommend appropriate actions • Build relationships with community medical providers and VNA services to enhance care coordination and act as a liaison between individuals, providers, and the treatment team Qualifications: • Associates degree in Nursing or equivalent • Experience working with adults with intellectual or developmental disabilities injuries • Valid Massachusetts drivers license • MAP certification preferred • Ability to work a flexible schedule in the areas of Braintree, Stoughton and North Attleboro Schedule: Monday-Friday, 8am-4pm or 9am-5pm Annual Salary: $85,000 USD - $85,000 USD At Eliot, we are committed to supporting you at every stage of your career, providing opportunities for growth, purpose, and community.
