From admission through 30 days post-discharge, our highly skilled, virtual interdisciplinary teams deliver personalized and comprehensive ‘wrap around care’ to break down the gamut of clinical, social & logistical barriers to post-discharge stability. This member-centric approach has already yielded substantial, sustained reductions in post-discharge complications and readmissions, and we’re continuously evolving our care model to keep more members safe at home. Required Skills and Experience Active RN license (compact/multi-state licensure preferred) Minimum 2 years of direct patient care experience in care management, population health, or transitions of care (virtual preferred) Minimum 2 years of people management experience, with a track record of team development and performance oversight Experience managing licensed care management teams Proven ability to drive clinical and operational outcomes through new initiatives and data-informed strategies Comfort with reviewing, interpreting, and acting on performance data Strong communication and collaboration skills, with a can-do attitude and a learning mindset A bit about the role We are currently seeking a TOC RN Manager to lead and support a team of RN Case Managers who deliver exceptional care and patient outcomes. This leader will help shape culture, performance, and clinical excellence by mentoring nurses, supporting their clinical decision-making, and ensuring care is delivered with compassion, integrity, and skill. As part of our interdisciplinary leadership team—alongside peer RN Managers and leaders from Nurse Practitioner, Care Coordinator, and Associate disciplines—you’ll collaborate on day-to-day operations, patient care strategy, and the continuous improvement of our virtual care model. Our ideal candidate is compassionate, solutions-driven, clinically strong, and experienced in people management. Responsibilities Include Coach and support RN Case Managers through 1:1 meetings, shadowing, audit reviews, and time studies Identify workflow challenges, contribute to cross-functional problem-solving, and help standardize best practices across teams Design and implement workflows, assessments, and protocols that improve efficiency and demonstrate patient impact Provide ongoing support to RN Case Managers by assisting with escalated or complex patient needs. Actively participate in clinical huddles and maintain a consistent presence in patient outreach efforts, dedicating at least 10% of time monthly to direct engagement Develop onboarding and ongoing education experiences for new and existing staff Collaborate with cross-functional leaders to align and optimize care coordination across programs Meet regularly with the Senior Manager of TOC to review performance metrics, surface risks, and celebrate team successes Attributes for Success A service-oriented leader who builds a culture of purpose, connection, and clinical excellence Skilled in coaching, best practice-sharing, and education, creating a learning-rich environment where nurses grow in both confidence and capability Inspires greatness—motivates and uplifts others to achieve their full potential while leading with compassion and clarity Proficient in performance monitoring and data analysis—comfortable using metrics and trends to guide team development and decision-making Exceptionally organized, proactive, and detail-oriented Comfortable working in a dynamic, fast-paced environment Curious and analytical—able to identify gaps, propose improvements, and implement with intent Highly collaborative, breaking down silos and embracing interdisciplinary partnership Committed to continuous learning and nimble leadership in an evolving care model Salary: $94,000 - 127,000 #LI-REMOTE The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
