About the position Responsibilities • Establishes and maintains rapport with Veterans and their family members/significant others, VA staff, other government and community agencies, CRC Sponsors, and the community. • Responsible for outreach and networking and recruiting CRC sponsors. • Acts as a liaison between VA and community resources, including outreach, networking, and public relation duties as needed. • Facilitates the most appropriate use of VA, other government agencies, and community resources on behalf of beneficiaries. • Develops working relationships and agreements with other organizations having responsibilities for the same patient population. • Maintains a current and productive network of referral resources. • Provides oversight for CRC staff and their use of community resources and linkages. • Serves as subject matter expert on CRC assessments and interventions. • Gathers data and interviews Veterans and their family members/significant others to assess the Veteran's situation. • Interprets and explains VA and community treatment options and available resources. • Receives and completes requests for services (consults) using advanced practice skills and expertise. • Provides consultation to Social Workers, interdisciplinary team members, and staff from other government and community agencies. • Develops psychosocial treatment plans in coordination with interdisciplinary team members. • Provides oversight and guidance to CRC staff providing treatment to Veterans in program. • Provides administrative and clinical supervision to senior social workers (case managers) assigned to the program. Requirements • Advanced practice skills in social work. • Experience in developing and coordinating procedures for use of community services. • Ability to assess Veterans' situations and develop psychosocial treatment plans. • Experience in providing oversight and guidance to staff.
