Posting Date 12/08/2025 2624 N Ankeny Blvd, Ankeny, Iowa, 50023-4704, United States of America Patient Coordination & Assignment - Manage the filtering process for identified super high utilizer ESKD patients to ensure appropriate alignment with the correct support pathway including Supportive Care, Licensed Clinical Social Workers (LCSWs) and HI-Care Pod. Coordinate and assign patients to HI-Care Pod RNs based on licensing, regional requirements, and patient needs. Maintain accurate and up-to-date records of all patient assignments, transitions, and care coordination activities. 25% Data Management & Tracking - Track project-related patient data, outcomes, and progress in a timely and organized manner. Support analysis of trends and outcomes to inform project goals and decision-making. Maintain project dashboards or trackers to ensure data accuracy and accessibility. 25% Communication & Education - Serve as a liaison between the project team and external partners to share educational information about the program’s goals and processes. Draft and distribute clear communication updates and reference materials to care teams and other stakeholders. Coordinate dissemination of training or informational materials related to the project. 20% Reporting & Presentation Support - Prepare and deliver regular progress reports and project status updates to stakeholders. Create PowerPoint slides, visual aids, and data summaries for leadership presentations. Support the documentation of project outcomes and key deliverables. 20% Operational Support - Schedule and facilitate project meetings, recording decisions and action items. Assist with workflow development and process improvement initiatives. Provide administrative and logistical support to ensure smooth project execution. 10% Qualifications - 2-4 years of experience Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook Required At least 2 years of experience in a medical/health-related office with healthcare insurance billing experience Required Prior experience coordinating care for a patient (e.g. referrals, ACO care management, etc.) Preferred Able to work autonomously with minimum supervision Required Able to effectively navigate EMR system without supervision Required Prior experience working in dialysis clinic and/or with renal patients Preferred Experience leading teams as a special project lead Required Travel: Up to 10% What We’ll Provide: More than just pay, our DaVita Rewards package connects teammates to what matters most.
