Job Description: • Develop and maintain transplant centers' partnerships, facilitating efficient and effective workflows between internal and external stakeholders • Support seamless care coordination by working with internal teams and promoting best practices for tracking patient journeys, such as facilitating EHR access. • Collaborate with transplant centers to break down barriers to transplantation and support outreach initiatives like 'Lobby Days.' • Organizing regular leadership calls, and expanding the Transplant Care Coordination (TCC) Memorandum of Understanding (MOU) • Implement standardized processes for engagement, such as patient pipeline calls, and support the growth of the Transplant Liaison program to provide patient support and strengthen partnerships.
