Active RN licensure, 3+ years nursing experience, 2+ years case management or home healthcare coordination, proficiency with Microsoft Office, and ability to work independently from home.
Description: • Coordinate care for Special Needs Plan (SNP) members with chronic medical and behavioral health conditions and SDoH needs • Conduct comprehensive assessments and develop individualized care plans • Provide education and disease management support • Collaborate with interdisciplinary care team and ensure accurate documentation • Identify and connect members with health plan benefits and community resources • Meet regulatory requirements and participate in team meetings • Support team through clinical decision-making and participate in projects and process improvements Requirements: • Active and unrestricted Registered Nurse (RN) licensure in the state of Missouri OR compact licensure in state of residence • Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated space to conduct work effectively • Confidence working at home / independent thinker • 3+ years of nursing experience • 2+ years of case management, discharge planning and/or home healthcare coordination experience • Familiarity with community resources and services • Excellent customer service and engagement skills • Ability to meet performance and productivity metrics • Experience or willingness to learn care management within Medicare and Medicaid managed care Benefits: • Affordable medical plan options, a 401(k) plan (including matching company contributions), and an employee stock purchase plan . • No-cost programs for all colleagues including wellness screenings, tobacco cessation and weight management programs, confidential counseling and financial coaching. • Benefit solutions that address the different needs and preferences of our colleagues including paid time off, flexible work schedules, family leave, dependent care resources, colleague assistance programs, tuition assistance, retiree medical access and many other benefits depending on eligibility.
