CuraLinc Healthcare is seeking a Special Accounts Care Coordinator. The Special Accounts Care Coordinator will be responsible for the case management of special programs requiring specific follow-up and connection to services. The Special Accounts Care Coordinator will ensure that members referred to the program are followed up with in a timely manner, have received their recommended referrals for higher levels of care, or routine outpatient care, and that billing and documentation match what each participant received. Primary Responsibilities • Outreach to Assigned Members in a Timely Manner • Complete the Required Number of Outreaches for Each Member Referred to the Program Per Guidelines • Ensure Documentation of Outreaches is Correctly Recorded on the Platform Associated with the Account • Provide Clinical Support and Recommendations to Members • Conduct a Comprehensive Clinical Assessment, Review Supporting Materials, Ensure Connection to the Appropriate Level of Care • Connect Members to the Correct Level of Care Based on Presentation and Need • Complete Assigned Outreaches and Referrals in a Timely Manner • Work Directly with the Team Leader that Oversees These Programs to Ensure Direct Communication and Supervision • Bill for Services Rendered Ensuring Billing is Correct • Ensure all Referrals are Viable Options for Each Participant Other • Attend Team Meetings as Scheduled • Assist with Training New Teammates • Attend Meetings with the Account Leaders as Needed • Other Duties as Assigned Qualifications • At Least 1 Year + EAP Experience • Clinical Licensure Required, LCSW, LCPC, LMFT • Strong Professional Written and Verbal Communication Skills • Must Have at Least 6 Months of Enhanced Case Management Experience • Strong Critical Thinking and Problem-Solving Skills • Case Management Experience a Plus • Strong Knowledge of Higher Levels of Care Preferred • Requires Knowledge of Different Mental Health Insurance Coverage Types and the Ability to Navigate Available Resources or Coverages to Assist Members This is a full-time, salaried role, located in the continental United States and Hawaii, and includes a robust benefits package, with 100% company-paid single tier medical coverage insurance, PTO, 401(k) with match, dental, vision, life, and disability insurance.
