Position: Nurse Case Manager (Hybrid) Location: Oakbrook Terrace, IL 60181 (Hybrid Remote) Schedule: Monday to Friday, 8-hour shift Type: Full-Time Pay: $34.00 - $36.00 per hour About The Role We are seeking a compassionate and experienced Nurse Case Manager to join our team. In this role, you will serve as a key advocate and care coordinator, developing and managing comprehensive care plans to promote the health and well-being of our patients. This hybrid position allows for a blend of on-site and remote work. Key Responsibilities • Develop, implement, and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams. • Act as the primary point of contact and advocate for patients and their families, facilitating access to medical providers and community resources. • Provide essential health education to patients and families to improve understanding and management of chronic conditions. • Conduct thorough assessments to identify unmet health and social needs. • Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication. • Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status. Qualifications • Required: • * Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois. • Minimum of 3 years of nursing experience. • Minimum of 2 years of experience with geriatric patients. • Ability to commute to and work on-site in Oakbrook Terrace, IL, as needed for a hybrid schedule. • Preferred: • * 2+ years of experience in Case Management or Care Coordination. • Experience in post-acute or long-term care settings.
