Both CHNw and DHS place high importance on continuing and advancing population health and value-based care to improve patient health outcomes. Make a Difference The Population Health Analyst supports the organization’s value-based care initiatives by collecting, analyzing, and interpreting data related to quality, utilization, and risk adjustment performance. This role transforms complex datasets into actionable insights that drive operational improvements and optimize outcomes across the population health continuum. The Analyst collaborates with leadership and all population health departments to monitor key metrics, evaluate interventions, and support performance reporting to health plans and regulatory bodies. Essential Duties and Responsibilities: • Collect, validate, and analyze clinical, claims, and operational data from EMR, data warehouses, and health plan feeds. • Translate data findings into clear, actionable recommendations for clinical and operational teams. • Identify members with open care gaps or chronic disease management opportunities. • Partner with operational teams to quantify ROI for population health initiatives. • Collaborate with analytics vendors or internal medical economics teams to enhance patient segmentation tools. • Present findings to technical and non-technical stakeholders. • Develop standardized templates and automated reports to streamline workflows. • Recommend workflow or process changes based on data insights. • Participate in cross-functional performance review meetings to discuss trends, barriers, and opportunities. • Perform additional related duties as assigned. Exceptional Skills and Qualifications Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving. • Associate degree in a related field required. • Two (2) years in healthcare analytics, preferably within value-based care, ACO, or managed care settings required. • Proficiency with SQL, Power BI/Tableau, and data visualization tools required • Working knowledge of claims data, EMR systems, and quality programs (HEDIS, STAR, ACO, CMS measures) is required. • Strong analytical skills. • Ability to communicate complex data insights clearly. • Proficiency in data visualization and reporting tools. • Knowledge of healthcare quality programs and regulatory requirements • This position will allow flexibility to work remotely. Community caregivers performing work remotely are permitted to live in the following states: Indiana, Illinois, Ohio, Michigan, Kentucky, Florida and Texas. Caregivers are not allowed to perform work remotely outside of the above states. Applicants from other states may apply; however, if hired, they will be required to relocate to one of the above states within 60 days of their employment date.
