Job Summary We are seeking compassionate and dedicated In Home Caregivers / Personal Care Assistants to provide exceptional support and care to clients in their own homes. This role involves assisting individuals with daily activities, ensuring safety, and promoting independence. Ideal candidates will have experience in senior care, home health, or working with individuals with disabilities, including dementia and developmental disabilities. The position offers an opportunity to make a meaningful difference in clients' lives through personalized care and support. Responsibilities • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting • Provide medication reminders and administer medications as prescribed, adhering to HIPAA regulations • Monitor vital signs and observe patient conditions to report changes promptly • Prepare nutritious meals tailored to individual dietary needs and assist with meal planning • Support mobility using equipment such as Hoyer lifts or resident lifts; assist with transfers and positioning • Maintain a clean and safe environment through light housekeeping tasks such as cleaning and laundry • Implement care plans designed by healthcare professionals, ensuring personalized and consistent care • Offer companionship and emotional support to promote mental well-being • Follow all safety protocols including CPR and first aid procedures in emergency situations • Document care activities accurately while respecting patient privacy and confidentiality Requirements • Prior experience in home & community care, assisted living, nursing homes, hospice, or long-term care preferred • Knowledge of Alzheimer's, dementia, memory care, autism spectrum disorder, developmental disabilities, and hospice care is a plus • Certifications such as CPR, First Aid, HIPAA compliance are highly desirable • Ability to work independently with minimal supervision while collaborating with healthcare teams when necessary • Strong observational skills for patient monitoring and reporting changes in condition • Physical ability to perform heavy lifting and assist with mobility devices like Hoyer lifts or resident lifts • Compassionate attitude with excellent communication skills for working with clients of diverse backgrounds • Valid driver’s license may be required for travel between client locations This role is vital in supporting individuals to live safely and comfortably within their homes while maintaining dignity and independence.
