Community Health Network has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care RN Case Manager. Coverage needs in the Havehill/Merrimack/Methuen/Gloucester/Amesbury/Newburyport MA areas. • Full-time, contract • Monday-Friday, 40 hours/week • Occasional weekends may be required • Highly competitive pay rates + IRS mileage reimbursement The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides. Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
