Company Description Chicago Hope and Palliative Healthcare, Inc. provides comprehensive hospice and palliative care services, including skilled nursing, certified nurse aide support, medical social work, therapy, spiritual care, bereavement counseling, volunteer services, durable medical equipment (DME), and respite care. We offer 24/7 clinical support, medication management, care coordination, and emergency response. Services are covered by Medicare, Medicaid, VA benefits, and most private insurance plans. Role Description This is a full-time on-site role for a Hospice Community Liaison located in Palos Hills, IL. The Hospice Community Liaison will be responsible for engaging with the community, conducting outreach activities, and building relationships with healthcare providers and organizations. The role involves communicating the benefits of hospice and palliative care services, providing exceptional customer service, and coordinating with home care services to ensure comprehensive patient support. Qualifications • Community Engagement and Community Outreach skills • Strong Communication and Customer Service skills • Experience in Home Care or similar field • Proven ability to build and maintain professional relationships • Excellent organizational and multitasking skills • Ability to work independently and as part of a team • Bachelor's degree in Healthcare Administration, Social Work, Nursing, or a related field is preferred
