The Clinical Leadership Associate will supervise and mentor primary care providers (PCPs) at our center. In this role, you will drive the execution of our core model by enhancing clinical quality, efficiency, outcomes, and satisfaction among clinicians and patients alike. Your time will be divided between direct patient care as a PCP and fulfilling essential leadership responsibilities. Your leadership will focus on a collaborative clinical-operations culture while ensuring the effectiveness of PCPs under your supervision. Key Responsibilities Include: • Providing care for patients with acute and chronic illnesses, particularly in older adults. • Taking full responsibility for patient care outcomes and coordinating consultations when necessary. • Collaborating with hospitalists to manage patient care in hospitals. • Conducting assessments, diagnoses, treatment, management, and health education for patients with complex chronic conditions. • Leading a care team, including medical assistants and coordinators, to optimize patient care. • Engaging with transitional care teams for patients unable to visit the office, ensuring comprehensive support. • Conducting weekly meetings to drive care transformations and flexibly assuming clinical leadership duties. • Actively participating in center management and supporting patient recruitment initiatives. • Coaching and mentoring PCPs to enhance clinical outcomes and foster team collaboration. • Partnering with operational leaders to boost center performance and membership growth. • Fulfilling additional responsibilities as assigned. Essential Skills and Qualifications: • Fluency in Microsoft Office Suite and other relevant applications. • Willingness to travel locally for center needs and nationally for training. • Strong English communication skills, with additional language fluency a plus. • Ability to exercise independent judgment in various scenarios. Education & Experience Requirements: • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related field. • Current, active medical licensure in your state is essential. • A minimum of one year of clinical experience; preference given to those experienced in geriatric or family practice settings. • Completion of ChenMed training programs as part of professional development. • Board certification in Internal Medicine, Family Medicine, Geriatrics, or qualifying specialty is preferred. • Current DEA number required for controlled substances. • Basic Life Support (BLS) certification must be obtained within 90 days of employment.
