The Patient Care Coordinator is responsible for managing clinical activities related to infusion patients, including supply management and coordinating services with nursing and operations. They also process medication refills under the supervision of a licensed pharmacist and maintain documentation in patient medical records.
A high school diploma or equivalent is required, along with experience in pharmacy customer service or as a pharmacy technician. Infusion pharmacy experience is preferred, and proficiency in MS Office is necessary.

Job Details: Responsible for performing select activities related to the clinical management of infusion patients including patient supply management, coordinating services with nursing and operations and processing refills under the supervision of a licensed pharmacist. Responsibilities Provide support to infusion pharmacy teams, pharmacy distribution and operations including but not limited to evaluating product usage, obtaining, and documenting supply inventories and specific supply needs, delivery and pick-up slip production, retrieval and distribution of various documents and equipment management all with-in mandated timeframes. Obtain medication profiles and patient updates/ assessments and refilling medications under the supervision of a licensed pharmacist. Providing appropriate and accurate documentation in the patient medical record. Maintain or exceed designated quality and production standards. Collaborate with Reimbursement Department and Distribution Department to ensure payor specific regulations/ guidelines are handled appropriately. I.E. Delivery restrictions or COD collection. Provide excellent internal and external customer service, maintaining a service orientation in all interpersonal relations that conveys a positive, professional image always. Act as a conduit of information between infusion pharmacy, operations, and the patient. Perform in accordance with system-wide competencies/ behaviors. Perform other duties/ tasks as required and assigned. Skills & Abilities Excellent customer service and communication skills. Knowledge and experience in homecare desirable, good prioritizing, multitasking, critical- thinking and time-management skills. Requirements High School diploma or equivalent required. Pharmacy customer service, pharmacy technician, and/or other related experience in an ancillary provider, or other relevant healthcare preferred. Infusion pharmacy experience preferred. Thorough knowledge in MS Office and PC skills required. Customer Service experience with communication via telephone, e-mail and/ or face to face required.
