SummitWest Care, a trusted non-profit home health agency in Grand Junction, is hiring a Weekend On-Call Triage LPN. This unique role offers a compact, high-impact schedule-ideal for LPNs wanting extra income, autonomy, and a mission-driven environment. You'll be the calm, clinical voice patients rely on when urgent needs arise after hours. Your Role During your weekend shift, you will: • Provide on-call triage coverage from Saturday 8:00 AM to Monday 8:00 AM • Respond to patient calls, assess clinical needs, and determine appropriate next steps • Perform in-person visits when clinically necessary • Collaborate with RNs, caregivers, and care teams to support urgent patient needs • Maintain accurate documentation of calls and visits • Work remotely when not in the field Compensation & Perks • $700 flat stipend for the weekend shift • $25–$30/hour for in-person visit time (based on experience) • Mileage reimbursement for all travel • Ability to work from home between field visits • Be part of a respected, community-focused non-profit organization What You Bring • Current LPN license in the State of Colorado • Home health experience preferred (not required) • Excellent clinical judgment and decision-making skills • Confident communicator with the ability to assess and triage independently • Reliable transportation, valid driver's license, and insurance • Comfortable working autonomously in patients' homes Why Choose SummitWest Care?
