Job Title: Patient Care Coordinator - Case Manager Locations: South San Francisco, CA Duration: 3 + Months (Possible Extension) Shift: 03.00 AM – 11.00 PM Rate: $70 per hour (Local) - $80 (Traveller) Job Responsibilities: · Responsible for assessing client needs, developing care plans, coordinating services, and advocating for clients to ensure they receive appropriate care and support. · Strong communication and interpersonal skills. · Ability to build rapport and trust with clients and their families. · Excellent organizational and time management skills. · Problem-solving and critical thinking abilities. · Familiarity with community resources and support networks. · Knowledge of case management principles and practices. · Ability to work independently and as part of a team. · Proficiency in relevant software and documentation systems. · Delegates tasks to support staff. · Performs other duties as assigned.
