BluePearl Pet Hospice is seeking an experienced Patient Care Coordinator to join our BluePearl Pet Hospice remote Patient Care Coordinator team. Requirement: Current Veterinary Technician License with 1+ year of Vet Med experience Are you committed and passionate about enriching lives through remarkable care for pets? The Patient Care Coordinator Pet Hospice Care acts as the primary point of contact between clients and hospice veterinarians, facilitating communication and guiding clients to the most appropriate choices of hospice and palliative care services. As the Pet Hospice Patient Care Coordinator, you will: ● Work closely with our interdisciplinary team to help with guidance and communication for current patients, including general email/phone communications, nursing care, or patient concerns. ● Assess and triages a patient’s clinical condition based on client history. ● Help clients assess their pet’s quality of life and provide support for pet loss. ● Provides virtual quality of life conversations with clients, understands treatment plans, presents details in a clear manner. ● Confirm appointments, prepares consent forms, answers and addresses client questions and concerns, performs callbacks, and ensures accurate communication and documentation with close attention to detail, while keeping the team informed of any pertinent appointment information. ● Aid in the development of others to competently perform skills in prerequisite levels. ● Support clients with discussions surrounding the euthanasia process. ● Approache and handles patients according to safety protocols and core values, compassionately, calmly, and gently. ● Aid in setting and driving a culture of patient safety and utilizes the Patient Safety Event Reporting accordingly. ● Assist with planning and coordination of daily service flow. ● Orient and educates pet owners and their families providing educational information regarding treatments, procedures, medications, and continuing care requirements. ● Connects clients with other associates to facilitate answers to questions. ● Coordinate information and care requirements for the pet; resolving issues that could affect smooth care progression; fostering team support for the pets' care and treatment. ● Document conversations with specialists and clients in the medical record; interacting with other services to expedite scheduling, procedures, and consults. ● Understand basic pharmacology of commonly used oral medications, including oral, injectable, and compounded. ● Discuss medications with clients. ● Maintain an advanced working knowledge of the pDVM portal, insurance policies, and procedures Why BluePearl?
