Summary Job Description: RN Clinical Coordinator Louisville Cardiology Full Time - Day Shift The RN Clinical Coordinator has a strong knowledge base in the care of cardiovascular patients and its related processes. This position will organize and coordinate the care of Louisville Cardiology’s patients for the physicians, and in doing so, will have frequent interaction with the patient and others involved in the patient’s care. Much of the work is done at the hospital bedside, but an important aspect is this position’s ability as a liaison to tie that work with the office. Important skill sets to possess are organizational, people-oriented, and self starter skills. Since this position represents, on a daily basis, our practice outside of the office, a strong sense of professionalism is vital. Some of the key accountabilities include: Hospital Process Organization and Coordination to ensure all patients are on rounds card; identify new patients and consults; verify patient room numbers; assign new patients to appropriate physician. Receive report from on call physician; report new and pertinent information to other physicians. Assist with the scheduling of procedures, especially inpatients; assist with coordination of physician schedules; coordinate hospital to hospital transfers with scheduling and admitting. Hospital Rounds: Triage patients and phone calls. Obtain H and P from new patients and consults and document in patient chart; chart follow up note in progress notes; prepare patients for discharge; follow up time, home meds and new prescriptions; obtain office records on established patients and place on chart. Provide education to patients on disease process, procedures, follow up visits, etc; assist with any other educational needs identified. Verbal and Written Communication and Documentation: Communicate all pertinent information to physicians in a timely manner; serve as liaison between hospital staff and physicians, as well as office staff and physicians. Assist in completion of medical records, including use of hospital internet based programs; assist in maintaining accurate procedure log, stress log and dictation problem log ; obtain records from other facilities as needed; assist office staff with computer based records retrieval. Document all phone calls (ER, office, floor) accurately. Research and Other Departments: Assist in the identification of patients for participation in research trials; other research duties as needed. Provide procedure list to Billing Department to help ensure accurate billing. Minimum Education, Training and Experience Required: Current and Active Kentucky RN License required. Must maintain an active BCLS and ACLS card through the American Heart Association. Knowledge of cardiovascular assessments, pathophysiology of cardiovascular disease, current medical and surgical treatments, and emergency procedures preferred.
