Manage referral coordination by acting as the point of contact for patients and physicians, monitor referral workqueues, and ensure timely, accurate patient care coordination.

Job Description: BHMG Patient Connect Hub Neurology The Patient Connection Hub (PCH) Referral Coordinator serves as the point of contact for patients, referring physicians and their office staff, and the various Baptist Health System departments for incoming and internal referral needs. The PCH Referral Coordinator will actively monitor and work all PCH Referral Workqueues in Epic. The goal is to deliver a high-quality and integrated Baptist experience; ultimately ensuring that patients receive the correct care, from the appropriate provider in the right timeframe. This role is expected to accomplish all work according to PCH guidelines while meeting reporting, metric and quality expectations. Minimum Requirements: High school diploma or equivalent required.
