Role Overview: As a Clinical - LTSS Service Care Manager, you will play a critical role in coordinating and managing long-term services and supports for adults over the age of 21. Your focus will be on delivering high-quality care plans that meet the unique needs of each client, ensuring their health and well-being in a community-based setting. What You'll Do: - You will conduct comprehensive assessments of clients’ health and social needs to develop individualized care plans. - You will coordinate with healthcare providers, families, and community resources to facilitate seamless service delivery. - You will monitor client progress and adjust care plans as necessary to achieve optimal outcomes. - You will provide education and support to clients and their families regarding available services and care options. - You will maintain accurate and timely documentation in compliance with regulatory standards. - You will participate in interdisciplinary team meetings to advocate for client needs and improve care strategies. - You will manage caseloads efficiently while ensuring high-quality service and client satisfaction. - You will respond promptly to client inquiries and emergencies, providing professional and empathetic support. What You Bring: - You have a minimum of 3 years of experience in clinical case management or a related healthcare role. - You hold a valid nursing license (RN or LPN) or relevant clinical certification. - You possess strong knowledge of long-term services and supports (LTSS) programs and regulations. - You demonstrate excellent communication and interpersonal skills to engage effectively with clients and multidisciplinary teams. - You are proficient in electronic health records (EHR) systems and documentation practices. - You have the ability to work independently in a remote/field setting within Hidalgo County, TX. Bonus Points If You Have: - Experience working with Medicaid or Medicare populations. - Bilingual skills in English and Spanish to better serve the local community. - Previous contract or remote work experience in healthcare. - Familiarity with community-based healthcare resources and social services.
