Role Overview: As a Clinical - LTSS Service Care Manager, you will play a critical role in coordinating and managing care for adults over the age of 21 who require long-term services and supports. This position involves working closely with clients, families, and healthcare providers to develop and implement care plans that promote independence and well-being. What You'll Do: - You will conduct comprehensive assessments of clients' health and social needs to develop individualized care plans. - You will coordinate services and resources to ensure clients receive appropriate and timely care. - You will monitor client progress and adjust care plans as necessary to meet changing needs. - You will collaborate with healthcare providers, community resources, and family members to support client goals. - You will document all care activities and maintain accurate records in compliance with regulatory standards. - You will provide education and support to clients and their families regarding care options and health management. - You will participate in team meetings and contribute to continuous quality improvement initiatives. - You will manage your schedule effectively to balance remote work and field visits within Hidalgo County. What You Bring: - Minimum of 3 years of experience in clinical care management, preferably in LTSS or related fields. - Licensed healthcare professional credentials (RN, LCSW, or equivalent) required. - Strong knowledge of long-term services and supports programs and regulations. - Excellent communication and interpersonal skills to engage effectively with clients and multidisciplinary teams. - Ability to work independently and manage time efficiently in a remote/field work setting. - Proficiency with electronic health records and care management software. Bonus Points If You Have: - Experience working in Hidalgo County or familiarity with local healthcare resources. - Bilingual skills in English and Spanish to better serve the community. - Previous contract or remote work experience. - Certification in case management or related specialties.
