Role Overview: As a Clinical - LTSS Service Care Manager, you will play a vital role in coordinating and managing care for adults over the age of 21 who require long-term services and supports. Your focus will be on ensuring clients receive comprehensive, person-centered care that promotes independence and well-being. What You'll Do: - You will assess client needs and develop individualized care plans tailored to their unique health and social requirements. - You will coordinate with healthcare providers, community resources, and family members to facilitate seamless service delivery. - You will monitor client progress and adjust care plans as necessary to meet evolving needs. - You will provide education and support to clients and their families to empower informed decision-making. - You will document all care activities and maintain accurate records in compliance with regulatory standards. - You will participate in team meetings and collaborate with multidisciplinary professionals to optimize care outcomes. - You will respond promptly to client inquiries and emergencies, ensuring timely interventions. - You will contribute to quality improvement initiatives aimed at enhancing service effectiveness. What You Bring: - Minimum of 3 years of experience in clinical care management, preferably within long-term services and supports (LTSS). - Licensed healthcare professional credentials (e.g., RN, LCSW, or equivalent) required. - Strong knowledge of care coordination, case management, and community resources. - Excellent communication and interpersonal skills to engage effectively with clients and stakeholders. - Ability to work independently in a remote/field setting while managing multiple priorities. - Proficiency with electronic health records (EHR) and documentation systems. Bonus Points If You Have: - Experience working in Hidalgo County or familiarity with local healthcare networks. - Bilingual skills in English and Spanish to better serve the community. - Previous contract or remote work experience. - Certification in care management or related specialties.
