Role Overview: As a Clinical - LTSS Service Care Manager, you will play a critical role in coordinating and managing long-term services and supports for adults over the age of 21. What You'll Do: - You will assess the needs of adults requiring long-term care and develop individualized care plans. - You will coordinate with healthcare providers, families, and community resources to ensure seamless service delivery. - You will monitor patient progress and adjust care plans as necessary to meet evolving needs. - You will provide education and support to patients and their families regarding care options and resources. - You will document all care activities accurately and maintain compliance with regulatory standards. - You will participate in team meetings and contribute to continuous improvement initiatives. - You will manage your schedule effectively to balance remote work and field visits within Hidalgo County. What You Bring: - Minimum of 3 years experience in clinical care management or a related healthcare role. - Strong knowledge of long-term services and supports (LTSS) and care coordination. - Excellent communication and interpersonal skills to engage with diverse populations. - Ability to work independently and manage time efficiently in a remote/field work setting. - Proficiency with electronic health records (EHR) and documentation standards. Bonus Points If You Have: - Experience working with adult populations in community-based settings. - Familiarity with Texas Medicaid and other state healthcare programs. - Certification in case management or related clinical credentials. - Bilingual skills, particularly Spanish-English, to better serve the local community.
