Coordinate patient care through assessments, care plan development, discharge planning, and collaboration with healthcare teams to improve patient outcomes. | Requires 3 years clinical nursing experience, RN license, healthcare industry experience, and proficiency with computer systems; case management experience preferred. | Overview We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. The ideal candidate will possess a strong background in nursing with expertise in critical care, case management, and patient services. This role is essential for coordinating patient care, ensuring effective discharge planning, and facilitating communication between patients, families, and healthcare providers. The Registered Nurse Case Manager will play a vital role in improving patient outcomes through comprehensive assessment and management of care plans. Responsibilities • Conduct thorough patient assessments to determine individual needs and develop personalized care plans. • Collaborate with multidisciplinary teams to coordinate patient services across various settings including inpatient, outpatient, and post-acute care. • Facilitate discharge planning by ensuring patients receive appropriate follow-up care and resources. • Monitor patient progress and adjust care plans as necessary based on clinical evaluations and feedback. • Maintain accurate medical records in compliance with HIPAA regulations and medical terminology standards. • Provide education to patients and families regarding health conditions, treatment options, and self-management strategies. • Engage in utilization management to ensure effective use of healthcare resources while maintaining quality of care. • Stay informed about best practices in case management, including knowledge of acute care, geriatrics, pediatrics, behavioral health, and infectious disease care. Requirements • 3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required. • Healthcare and/or managed care industry experience. • Case Management experience preferred-- Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding #INDPM Job Type: Full-time Pay: $35.93 per hour Expected hours: 40 per week Benefits: • Dental insurance • Health insurance • Vision insurance Experience: • Clinical: 3 years (Required) • Managed care: 1 year (Preferred) License/Certification: • RN License (Required) Work Location: Remote
