Perform cleaning, maintenance, and minor repairs to ensure a safe and hygienic environment in assigned areas. | High school diploma preferred, prior janitorial experience beneficial but not required, ability to operate cleaning equipment, physical stamina, and good communication skills. | Responsibilities: • Sweep, mop, and vacuum floors in all assigned areas, including hallways, stairwells, offices, and common spaces. • Clean and disinfect restrooms, ensuring supplies (soap, paper towels, toilet paper) are adequately stocked. • Wipe down surfaces, including desks, countertops, windows, and mirrors. • Empty trash and recycling bins, replacing liners as needed. • Remove and dispose of waste according to facility guidelines. • Monitor and report any maintenance issues such as leaks, broken equipment, or facility damage. • Perform basic repair tasks such as replacing light bulbs and minor fixture repairs. • Ensure cleaning equipment and tools are properly maintained and stored after use. • Follow proper cleaning protocols to ensure a safe and hygienic environment. • Use appropriate cleaning products, equipment, and protective gear. • Maintain compliance with health and safety regulations, reporting any hazards to supervisors. • Assist with deep cleaning tasks, such as carpet cleaning, floor waxing, and window washing. • Support special event setup and breakdown, including moving furniture or equipment. • Participate in seasonal cleaning tasks as required (e.g., snow removal from entrances). Manage and maintain a caseload of clients with HIV/AIDS and related conditions, conduct outreach and enrollments, provide follow-up services, document care activities, and participate in quality improvement and training. | Requires a degree or certification in health or human services, experience or interest in HIV/AIDS and related social issues, bilingual skills preferred, strong communication and organizational skills, and ability to work full-time. | Overview The Care Manager II is responsible for independently enrolling a minimum of 3 cases per month (36 cases a year); Provide follow-up services to meet the client’s needs and tracking for their caseload. Case load volume will be determined by the Administration of HIV, Health Home and Behavioral Services, adjustments to case load will be made according to agency need. Must use discretion and independent judgment in exercising duties. Responsibilities Be individually responsible for Outreach and Enroll (bottom up) a minimum of 3 cases per month. Additional clients may be assigned through top-down referrals to assist in building case load; however, they are not included in the 3 cases per month. Replace every closed case with a new case to maintain case load which is not included in the 3 cases per month. Maintain full responsibility for caseload including assessments, HARP Eligibility assessments, Care Plans, HML’s, timely documentation; face to face contacts at a minimum of once every 3 months; Conduct home visits and fieldwork on an ongoing basis and as needed; Conduct outreach activities for both missing and new clients; Case conference with and obtain necessary records from all primary agencies that are involved with the clients. Provide appropriate referrals and ensure follow-up by monitoring the quality of services, verifying and ensuring client participation; Provide education and supportive counseling to ensure that clients understand and follow up with services to which they are referred Ensure that ALL needed services are addressed /delivered for each client monthly without limiting services to a single provision of care; services should be prioritized and specific to clients’ needs and not prescriptive. Ensure every client on their caseload is CONTACTED AND BILLED FOR each month and provide weekly tracking of billing to their supervisor. Document the billable and non-billable services in the required software within 24 hours. Be specific and include comprehensive notes for every service provided. Track accurate and up-to-date case lists including closures in a timely manner Engage top-down referrals within 24 hours of receipt; Complete enrollments in a timely manner, track, and submit immediately to the Care Manager supervisor assigned Participate in the agency quality improvement and professional development programs, attending internal and external training courses and committees Administrative Responsibilities: All aspects of the care management process to ensure timely and accurate completion of all care management activities Ensure that documentation is completed in a timely manner, including progress notes written within 24 hours. Attend weekly care management meetings facilitated by the Care Manager supervisor Work with your supervisor to ensure that your caseload is covered when you are out of the office. Other Responsibilities: Available for evening and weekend telephone crisis intervention and coverage for other staff as needed If bilingual, translate for non-English speaking clients Additional duties as assigned Qualifications Master's or bachelor's degree preferred in health or human Services-related fields or Associate’s degree in health or human services related fields, or certification as an R.N. or L.P.N. Bilingual Spanish/English preferred. Experience or interest working with HIV/AIDS, chronic illnesses substance abuse, mental illness and homelessness. Awareness of and sensitivity to cultural and socioeconomic characteristics of populations served. Ability to work collaboratively with other professionals. Ability to set appropriate limits with clients and co-workers. Excellent writing and oral communication skills. Good management and organizational skills. Basic computer skills required. Able to work 35 hours per week, Monday through Friday during normal business hours, or as needed to carry out the job responsibilities.
