We are focused on personalized integrative care and are dedicated to changing the lives we serve. We are seeking a dedicated Care Manager to join our Watkins team. The ideal candidate will have a passion for helping others and possess a diverse skill set in areas such as motivational interviewing, childcare, addiction counseling, behavioral health, crisis intervention, Medicare knowledge, supervising, workforce development, crisis management, and Section 8 assistance. Essential Functions: • Provide comprehensive care management services to clients in need • Conduct assessments to identify client needs and develop individualized care plans • Utilize motivational interviewing techniques to engage clients in their care • Coordinate with external agencies and resources to support client needs • Offer addiction counseling and behavioral health support as needed • Supervise and mentor staff members to ensure quality service delivery • Assist clients in navigating Medicare benefits and entitlements • Implement crisis intervention strategies when necessary Oversee workforce development initiatives for client empowerment • Provide guidance on Section 8 housing options for eligible individuals Education and Experience Requirements: • Associate Degree, preferred. • Must have at least 4 years of Case Management Experience • Proficiency in motivational interviewing techniques • Knowledge of addiction counseling principles • Strong background in behavioral health services • Ability to handle crisis intervention situations effectively • Familiarity with Medicare regulations and benefits • Experience in workforce development programs • Proficient in crisis management strategies • Understanding of Section 8 housing programs This position offers a rewarding opportunity to make a positive impact on the lives of those in need.
