Shift Details: Morning 7a - 3P / Evening 3P - 11P / Night 11P - 7A Mon-Fri; Every other weekend required The Resident Care Associate reports directly to the Family Experience Director, Pearl Director or the Assistant Family Experience Director. PURPOSE Resident Care Associate | CNA The Resident Care Associate is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living and Memory care neighborhoods. This role includes providing hands on care, physical and emotional support as outlined in each resident’s Personalized Service Plan (PSP) while maintaining a safe, comfortable home like environment. The Resident Care Associate is responsible for demonstrating all elements of wellness, remaining in accordance with federal, state and local standards and regulations and Phoenix policies to promote the highest degree of quality care and services to our residents. PRINCIPLE DUTIES AND RESPONSIBILITIES RESIDENT CARE Resident Care Associate | CNA Participates in the development of the Personalized Service Plans and updates; attends service plan review meetings with families as requested Follows the hydration schedule established for the residents is maintained during the shift Completes resident meal attendance sheets have been completed and any resident who has not dined in the dining room has been provided room service and / or accounted for. Knows and complies with all resident rights Initiates, assigns, and assist with activities, as appropriate Check all assigned residents daily for cleanliness, appropriate clothing, any continence issues and change in condition Uses proper sanitary procedures and universal precautions Checks on assigned resident rooms daily for general cleanliness and neatness and takes appropriate action; ensures high risk rooms are double checked Assist with conducting laundry rooms, common areas – including bathrooms – checks for cleanliness and any exposed hazardous chemicals; and takes appropriate action (i.e. contacting housekeeping, personally cleaning the room, securing the chemicals) Completes all ADLs for assigned residents according to the resident’s Personalized Service Plan (PSP) Responds to security system and resident call bells promptly & immediately; takes appropriate action including resetting call bells EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION Resident Care Associate | CNA High School education preferred and may be required by the state Previous experience working with elders or disabled individuals, preferred Must be at least 18 years of age to perform the personal care aspects of the RCA position Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times

Shift Details Activities Mon-Fri ; Every other weekend required The Assistant Program and Outreach Director reports directly to Program and Outreach Director. SUMMARY The Assistant Program and Outreach Director is responsible for assisting in the day to day delivery of resident centered activities that promotes and integrates the Phoenix Senior Living philosophy to incorporate wellness in all aspects of the seniors’ lives we serve. Ensuring all dimensions (emotional, sense of purpose, physical, social, intellectual and spiritual) are incorporated into the services we provide and embrace. Requirements: RESPONSIBILITIES Activities • Refers to the Resident Profile and addendum for every new resident • Assist with facilitating a variety of appropriate and personalized activities which incorporates all dimensions wellness and are available throughout the day and evening with resident care associate participation • Understands the recognition of resident changes in condition, takes appropriate action • Assist residents to and from activity room (s) when necessary • In memory care assist the residents to and from Life Skills and other normalizing life enriching activities and routine and assist with engagement • Reviews, reads, notates, and initializes communication log to document and learn about pertinent information and any resident’s physical and behavioral changes • Maintains activity areas in an orderly manner • As applicable, assist with the delegation of daily care of any animal and or plants within the activities program and services, as well as maintenance of records related to this care • Assist in maintaining an inventory of activity and programming supplies, games, programs and crafts EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION Activities • Education: High School Diploma, and job related experience preferred in a senior living community. • Ability to lead activities and motivate volunteers and fellow associates for their involvement in the social events and various scheduled social activities • Knowledge and experience in Assisted Living industry and Dementia care preferred

Provide medication assistance and resident care while supporting wellness and clinical quality assurance in an assisted living environment. | Certified Medication Aide certification, at least one year experience with elders or disabled individuals, leadership skills, and knowledge of assisted living or dementia care. | Description: Community Name Phoenix at James Creek Shift Details Med Tech / Certified Medication Aide Mon- Fri; Every other weekend The Med Tech is responsible for monitoring the health and well-being of our residents and provides leadership support and assistance with resident care and services. Responsibilities include but are not limited to medication care assistance/administration, communication with resident support services and families and maintaining clinical quality assurance while leading, demonstrating and supporting all the elements of wellness which are core components of Phoenix programs and services. PRINCIPLE DUTIES AND RESPONSIBILITIES Med Tech / Certified Medication Aide Resident Care • Identifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process • Ensures proper follow through and documentation for residents with a change in clinical care needs • Supports the Wellness Nurse with Completing the clinical sections of monthly wellness visits and PSP to accurately reflect the resident on a monthly basis • Informs the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out • Assist with obtaining weights and vital signs monthly for each resident prior to completion of monthly wellness visit • Assist with resident care as needed Requirements: EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION Med Tech / Certified Medication Aide • Must be certified Medication Aide and/or meet the state requirements for medication aide • A minimum of one year of experience working with elders or disabled individuals, preferred • Must have demonstrated Leadership capabilities • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred • Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
