Assess, plan, and evaluate patient care for homebound patients, including ordering tests and medications, and coordinating referrals. | Licensed in Texas, graduate from an accredited program, board certified or eligible, with 2+ years outpatient primary care experience. | Job Summary: This position works under the supervision of the Managing Physician for Primary Care at Home to help provide value based medical care and care coordination for homebound patients. The APC is involved in the planning, implementation, and evaluation of medical and nursing care for patients with complex care needs. The Care Manager - Nurse Practitioner assesses the physical and psycho-social condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories. The Care Manager orders, collects, interprets, and evaluates laboratory and other diagnostic tests to assess patient problems and health care needs. The Care Manager may prescribe and/or recommend medical drug therapies or other forms of related therapeutic treatments and procedures in accordance with approved protocols/policy and with attention to efficacy, safety, and cost. The Care Manager formulates and implements comprehensive outcomes-based patient care plans based on patient assessments, scientific rationale, standards of care, and professional practice guidelines. The Care Manger initiates timely consultations and referrals when the problem exceeds their scope of practice and/or expertise. This position requires home and virtual visits. The goal is to do about 6 home visits per day. Kelsey Seybold Clinic accepts H1B Visas. Responsibilities: Serve as physician extender in the clinic setting Evaluate patients in the clinic setting Document evaluations in a timely manner Order medications and tests as per guidelines and protocols Follow-up on tests and consults Schedule clinical studies as per guidelines and protocols Coordinate referrals/consults Handle MKO messages, results, CC charts, telephone encounters and surgical risk assessments as needed within scope of practice and as directed by the supervising physician Schedule clinical studies as per guidelines and protocols Coordinate referrals/consults Qualifications: Graduate from an Accredited School of Physician Assistant Studies or Master’s in Nursing. Licensed in the State of Texas. Board Certified and/or Board Eligible. 2 years of experience in an outpatient primary care setting required.
