Provide home- and community-based visits to support housing stability, wellness, and recovery. Deliver crisis intervention, behavioral health symptom management, and de-escalation for clients with mental health and substance use challenges. Facilitate peer support and therapeutic groups aimed at promoting wellbeing and life skills. Collaborate with case managers and other providers to coordinate services, monitor client progress, and address complex clinical needs.
