Make a Difference The Hospice Registered Nurse is responsible for case management of a group of hospice care clients, providing direct nursing care, and coordinating and supervising other disciplines involved to promote quality services to patients. The Hospice RN is responsible for providing skilled care to assigned patient according to the plan of care established by the patient's physician. The Hospice RN practices within the scope of nursing as defined by the Nurse Practice Act. This position will cover the south/southeast which is I-70 south (some of Marion County and all of Johnson County). Would cover areas such as New Palestine, Beech Grove and Greenwood. Exceptional Skills and Qualifications -Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. -Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). -Bachelor of Science in Nursing preferred. -One year nursing experience required. -Home care/hospice experience preferred. -CPR certified Why Community? Bachelor’s degree in nursing or Master’s degree in social work is required. Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana, as listed in the Nurse Licensure Compact (NLC) is required. 3 or more years of clinical staff management experience in population management, managed care/insurance, or hospital case management department setting is preferred. 3 or more years of care management experience in a clinical setting is preferred. 5 or more years of experience leading and managing operations across multiple markets is preferred.
