Lead and manage application and data migration projects in a Care Case Management environment ensuring compliance with HIPAA and effective communication with stakeholders. | Requires strong technical project management experience in healthcare/social services, hands-on HIPAA and EHR/EMR knowledge, and excellent communication and analytical skills. | Responsibilities: • Collaborate with stakeholders (case managers, families, support providers, end users) to ensure project success. • Identify requirements, perform gap analysis, and manage end-user communication. • Lead application and data migration projects within a Care Case Management environment. • Ensure compliance with HIPAA regulations, EHR/EMR systems, and patient case management. • Manage project schedules, status reports, meeting agendas, and facilitate meetings/testing. • Communicate effectively with business owners, IT teams, and management. Qualifications: • Strong technical project management background in Care Case Management (social work, health & human services, hospital settings). • Hands-on experience with HIPAA-constrained data, EHR/EMR, and patient case management systems. • Proven experience managing application & data migration projects. • Excellent communication (verbal & written), analytical, and decision-making skills. • Proficiency in PowerPoint, Excel, and strong time management abilities. • Experience with technology-driven change management. Nice to Have: • Experience with Monday.com project management tool. • Knowledge of person-centered practices/strategies in healthcare or human services.
