Build and lead care teams, develop workflows, and implement AI-driven solutions to improve patient engagement and operational efficiency. | Extensive experience in healthcare delivery operations, team building, automation, and cross-functional leadership, with a focus on value-based care. | Job Description: • Build & Lead Next-Generation Care Teams • Recruit, train, and scale care teams that proactively engage patients through outbound and inbound outreach. • Drive excellence in communication, documentation, and value-based outcomes. • Develop workflows for triage, care coordination, education, and escalation. • Lead the adoption of AI-driven outreach, documentation, quality monitoring, and decision support. • Ensure care teams are working at top-of-license, shifting repetitive tasks to automation. • Partner with Product and Data teams to deploy, test, and scale automation that improves efficiency, accuracy, and consistency. • Own day-to-day care operations: performance dashboards, staffing models, QA programs, and capacity planning. • Ensure delivery against contractual SLAs and key payer metrics. • Translate payer requirements into clear operational workflows and accountability structures. • Stand up care teams with the right structure, capacity, and leadership coverage for live operations. • Launch operating standards for outreach, documentation, and quality. • Activate live care programs integrated with Chamber’s platform. • Implement first-wave AI functionality into frontline workflows to demonstrate efficiency gains. • Lead an exec-level performance readout on engagement, capacity, and scalability readiness.
