Responsibilities • Provide skilled nursing care in a home health setting • Travel to patient homes within the coverage area: Howard City, Six Lakes, Tufant, Cedar Springs, Rockford, Belding, and Saranac • Perform patient assessments, develop and update care plans, and coordinate with healthcare teams • Educate patients and families on disease management and health maintenance • Document visits and care in compliance with regulatory standards Requirements • Active Registered Nurse (RN) license – Michigan State Board • Minimum 1 year of nursing experience (Home Health experience preferred) • Strong clinical, communication, and organizational skills • Ability to work independently and manage patient visits across multiple counties Equipment & Software Needed • Tablet, phone, nurse bag (provided or required per facility policy) • Access/knowledge of: Forcura, Swift, HCHB
