Description Join Our Mission: Astor Services' Rockland Adolescent Partial Hospitalization Program (PHP) is seeking a proactive, highly motivated, and organized Administrative Case Manager to support a dynamic, collaborative team dedicated to helping youth remain out of inpatient hospitalization. This role is perfect for someone who thrives in a fast-paced environment and wants to make a meaningful impact on families and communities. What You'll Do: As an Administrative Case Manager, you will play a vital role in ensuring smooth operations and exceptional care. Your responsibilities include: • Office Support: Provide general administrative assistance to PHP staff. • Referral Management: Schedule intakes, maintain the client census board, and track referral/admission data. • Preadmission Coordination: Enter clients into CareLogic and complete intake consents. • Data Management: Maintain program data required by OMH, local county, and Astor. • Family & Community Liaison: Serve as a key contact between families, agency personnel, and community providers. • Surveys & Screenings: Complete Social Care Network screenings and satisfaction surveys with caregivers/guardians. • Environment of Care: Assist the Program Supervisor in managing the program environment and ensure compliance with documentation and reporting requirements. • Special Projects: Assist with assignments identified by the PHP Supervisor or Director.
