West + potential minimal travel Position Type: Full-time, 40 hours/week Compensation: Starting at $18/hour (Certified Medical Assistant required). Our Chronic Care Management (CCM) company partners with clinics to provide essential support to patients managing chronic conditions. As a Patient Coordinator, you'll play a critical role in helping patients enroll in our CCM program, ensuring they receive the care they need to live healthier lives. About Chronic Care Management: CCM is a Medicare-sponsored program designed to improve health outcomes for patients with chronic conditions by coordinating care, offering ongoing support, and enhancing communication between providers and patients. At Assurance, we consistently celebrate stories of patients for which we have helped prevent hospitalizations, identified harmful drug interactions, and even assisted with Social Services. Key Responsibilities: • Patient Engagement: Professionally and convincingly explain the benefits of the CCM program to patients and enroll them in the program. • Technology Proficiency: use our software platform accurately to input patient data and complete enrollment processes. • Adaptability: Navigate varying clinic etiquettes, policies, and workflows while maintaining professionalism. • Task Management: Balance multiple priorities, follow checklists, and complete tasks thoroughly and on time. • Professional Communication: Collaborate effectively with clinics staff, patients, and colleagues, and respond promptly to emails.
