Full Description

The Mental Health Case Manager is a direct service position and is responsible for providing services to youth, adults and older adults, including case management, wellness/recovery services, mental health services, resource development, service linkage and service coordination for the Transcultural Wellness Center (TWC), a community based, outpatient clinic. Bachelor’s Degree or AA degree in Social Work, Psychology, Child and Family Studies, Sociology or related field. Valid California Driver's license and automobile liability insurance. ESSENTIAL DUTIES • Provide case management, assessment and service coordination to identify and develop community resources, assist the client to develop natural support systems for themselves and their families, make referrals, and assist with service linkage. • Assist in completing paperwork to assess presenting mental health symptoms, current social and life problems and/or risk factors. • Plan ahead, set priorities and schedule clients regularly to help clients reach their treatment goals. • Provide psychosocial rehabilitation activities and support groups to enhance the client’s ability to better function within their community and decrease isolation. • Utilize services for language interpretation to provide services for individuals and families to facilitate accurate understanding of client problems and appropriate service planning and access. • Provide services at home or in the community as needed to facilitate engagement, to help the client reach their treatment goals, to teach, train and develop functional and independent living skills, to promote resiliency, empowerment and rehab recovery. • Meet productivity and billing targets and documentation requirements. • Communicate and collaborate with the client’s treatment team members including supervisors, clinicians, doctors, and other clinical staff to maintain coordination of care for the client. • Provide crisis intervention and after-hours assistant care as requested with clinician support. • Provide suicide prevention support and safety plan when client is in crisis. • Attend and participate in staff trainings and agency meetings, weekly supervision and team meetings as scheduled. • Perform other related duties as assigned. Send resume and cover letter to Carolyn Yang at cyang@apccounseling.org REQUIRED KNOWLEDGE AND SKILLS • Ability to engage clients in rehabilitation recovery planning and activities. • Ability to effectively conduct a risk assessment and safety planning if the client is in danger of harming themselves or others. • Knowledge of or willingness to learn how best to provide employment and housing support services and housing placements. • Ability to document appropriately and thoroughly and to maintain confidentiality. • Ability to work in an environment that is client-oriented, strength-based and values diversity, culturally and spiritually. • Ability to exercise appropriate judgment and decision-making. • Strong interpersonal communication skills to maintain effective working relationships with staff and community partners. • Ability to follow procedures and behave in a professional manner without constant supervision. MINIMUM QUALIFICATIONS • Bachelor’s degree or AA degree in Social Work, Psychology, Child and Family Studies, Sociology or in a related field preferred. • 2 years of direct service experience in a mental health setting preferred. • Valid California Driver’s license and automobile liability insurance. • Computer skills including Microsoft Office or Google Suite Job Type: Full-time Pay: $25.00 - $26.00 per hour Expected hours: 40 per week Benefits: • 403(b) • 403(b) matching • Dental insurance • Health insurance • Life insurance • Paid time off • Vision insurance Application Question(s): • Must pass background check • Must have valid California driver's license and automobile insurance Work Location: In person
